Find your forms, choose the right type and follow the guide to set it up.

Use forms to collect information from your customers and learners. You can ask questions while someone books a course, or share a separate form for enquiries and applications.

This guide helps course providers choose the right type. Each type has its own guide with the full instructions and settings.


Find your forms

Open your Cademy dashboard and select Forms in the left-hand menu. Your existing checkout forms are here too.

You need the Course Owner or System Admin role to manage forms. If the menu is missing, ask the person who manages your team’s access.


Choose a form type

General forms

Use a General form when you want someone to send you information without booking a course. For example, collect a training enquiry or ask which workshops they are interested in.

Share the form with a link or put it on your website. You can then read and manage the answers in Cademy. Sending a General form does not create a course booking.

Follow Forms: General to create the form, share it, set up emails and manage responses.

Checkout forms

Use a Checkout form for the questions people answer while booking a course. For example, collect the booker’s contact details, each participant’s dietary requirements or the business they are booking for.

Choose whether to collect details just for the booker or for every participant, and whether to offer personal or business registration.

Follow Forms: Checkout to set up the forms and course settings.


Create or edit a form

  1. In Forms, open an existing form or click New Form.

  2. For a new form, choose its type, enter a Name your team will recognise and click Create Form.

  3. Open Form Editor to add and arrange questions. A field is a question or a space for information, such as a name.

  4. Click Save to keep your changes. Then follow the guide for that form type to make it available to people.

You cannot change a form’s type after creating it. The tabs and settings depend on the type, so use its own guide for publishing, course setup and managing answers.


Frequently asked questions

Where have my old checkout forms gone?
They are listed under Forms. You do not need to recreate them.

Why does my form have different tabs from another one?
The type controls what you can do. General forms collect separate responses; Checkout forms are used during bookings. Follow the guide for your type.

Can I make a copy instead of changing the original?
Yes. In Forms, open the three-dot menu beside it and choose Duplicate…. The copy keeps the same type. Follow that type’s guide to check its settings and put the copy into use.
