Add Forms to Lessons

Add General or Feedback forms to online lessons, choose submission requirements and find learner responses.

Use this guide if you are a System Admin or Course Owner adding forms to on-demand or blended lessons hosted in Cademy. Learners can answer questions inside a lesson, and you can decide whether submission is needed before they finish it.

A Form block places an existing form inside a lesson. A response is the saved set of answers. A Registration is one learner's place on a course or course date.

Before you start

  • Use the block-based course content editor. Older courses using the legacy lesson editor do not have Form blocks. If Form is missing from the lesson block menu, ask Cademy support to check which editor your course uses.

  • You need a General or Feedback form. Checkout forms and archived forms cannot be selected. Feedback forms must be enabled for your account.

  • Prepare the questions in Forms and select Save. See Forms: General or Forms: Feedback and Course Reviews for field types and settings.

A General form can stay in Draft for use in lessons. You do not need to publish a public form link, and lesson use does not use a published General-form slot. The course and lesson still need to be available to the intended learner.

Add a form to a lesson

  1. Sign in at admin.cademy.io and select Courses.

  2. Open the on-demand or blended course. Select Edit Course, then Contents.

  3. Open the lesson that should contain the form. For lesson creation and publishing, see Create and Edit Lessons and Course Content.

  4. On an empty line at the main lesson level, type / and select Form. Do not put it inside columns, a table, a toggle or another container.

  5. Select Choose a Form. Choose from General Forms or Feedback Forms. Use Search forms… if needed.

  6. Check the selected form and set its requirements using the options below. Select the lesson's main Save button.

The Draft badge identifies a General form whose public link is not published. Course default identifies the course's usual Feedback form; you can select a different Feedback form for this block.

If you need a form first, Create New Form opens Forms in another tab. Create and save it, return to the lesson, then reopen the picker. Edit Form opens the selected form in another tab. Changes to that form apply everywhere it is used.

Choose whether learners must submit

The form must remain available for submission requirements and later-lesson locks to apply. A missing or archived form cannot be shown to learners and does not block their progress. Replace an unavailable form before relying on it to collect answers.

Both switches start off. Choose the behaviour for this lesson:

  • Students must submit this form to finish the lesson: turn this on to require submission before the learner can complete this lesson. On its own, this does not lock later lessons.

  • Also lock later lessons until it's submitted: after enabling the first switch, turn this on to keep later lessons locked. The learner must submit the form and complete the lesson containing it before that lock clears.

Leave both switches off for an optional form. Other lesson schedules, quizzes and SCORM requirements still apply. Turning off the first switch also turns off the later-lesson lock. Select Save after changing these settings.

Example: Place a Feedback form in the final lesson and require submission. The learner answers it, submits, then completes that lesson. For an earlier information checkpoint that must be finished before the rest of the course, turn on both switches.

What learners do

Learners open the lesson through their course. General forms use their saved contact details where available, and their account email is filled in and locked.

  1. Complete the questions and select the form's submit button. Its wording is set in the form editor.

  2. Wait for the saved receipt showing the submitted answers and date. Learners cannot edit or resubmit the response from the lesson.

  3. Complete any other required lesson activities, then select Complete & Continue, or Mark as Completed in the final lesson.

Before a required form is submitted, the lesson completion button can show Submit the Form to Continue. A required quiz or SCORM activity can take priority in that message. Submitting a form does not complete the lesson by itself.

Inside a lesson, a General form shows the saved response instead of its public thank-you message or redirect. General form contact updates, emails, team notifications and Inbox handling follow the form's settings. Feedback forms use the course-feedback process and its public-review rules. See the form-type guides for those settings.

When a Feedback response is recorded against the Registration, Cademy skips the automatic feedback request on course completion. A General response does not count as course feedback. A separate day-after email is controlled by its own settings.

Find and export submitted answers

System Admins and Course Owners can review answers in Forms:

  1. In the Admin dashboard, select Forms and open the form used in the lesson.

  2. Select Responses, then use Source > Lesson to show lesson submissions. Clear the search or filters if an expected response is missing.

  3. Select a response to read its answers and course and lesson details. For General forms, use Columns > Lessons > Course to show the course in the table.

  4. To download responses, set the date and Source filters you need, select Export, then Download CSV when ready. General form exports include Course and Lesson columns. Feedback exports include Course, Course Date and Facilitators, but no Lesson column.

The export uses filters, not the name or email search text. Uploaded files are not included in the CSV; open the response to download them.

To check one learner, open Registrations, select their Registration, then expand Lesson Progress and the section containing the lesson. A Form Submission entry and Submitted badge identify its saved response. Hover over Submitted to see the form name and submission date. System Admins and Course Owners can select Form Submission to open the response. Other team members with access to the Registration can see the submission entry but cannot open the form response.

Reuse, copy or remove forms

The same form has one response per Registration, even when used in several lessons. Once submitted, its saved response is reused. Choose separate forms if you need fresh answers at different points in the course.

Duplicating or copying a lesson keeps its selected form and both requirement switches. Duplicating a course also keeps the shared form. These actions do not create a separate copy of the form. Editing that form changes it wherever it is used.

To create a separate form, open Forms, open the form's three-dot menu and select Duplicate…. Enter a Name, select Duplicate, and check the copy's questions and settings. Return to the lesson and select the copy in the Form picker, then save the lesson. Shared Contact fields still refer to the same field, even in a copied General form.

Before deleting a form, open Forms > the form > Usage. Open each listed lesson, remove or replace its Form block, then save. Cademy prevents deletion or archiving while a lesson uses the form. Feedback forms can also have course or account-default uses to replace. Export any responses you need before deleting.

Unpublishing a General form closes its public link and website embeds. It remains usable in lessons as a draft. Remove or replace the Form block and save to stop a lesson using it.

Preview and check completion

Preview as Student lets you inspect the saved lesson, but forms show Preview. Responses aren't saved. and cannot be submitted. Use a real test Registration to check submission, receipts, lesson completion and later-lesson locks. Follow Test a Course as a Learner.

An automatic certificate still depends on course completion and the course's certificate settings. Blended courses also require attendance. A form submission alone does not issue a certificate. See Certificates.

An administrator can correct a lesson to Completed without submitting its form. That does not create a response. Check Form Submission or the form's Responses tab when you need the actual answers. See Change a Learner's Lesson Progress.

Frequently asked questions

Why is my form missing from the picker?
Check that it is General or Feedback and is not archived. Checkout forms are excluded. If Feedback is unavailable throughout your account, ask Cademy support to check access. If you just created the form in another tab, save it and reopen the lesson's picker.

​

Why is a later lesson still locked after submission?
The learner must also complete the lesson containing the form. Check any other required quiz or SCORM activity in that lesson, then use Complete & Continue. Other release schedules and earlier lesson requirements can still restrict access.

​

What if the editor says the form is unavailable or archived?
Use Choose Another Form to select an available replacement, then save the lesson. A missing or archived form cannot be shown to learners and no longer blocks their progress. Replace it before relying on that lesson to collect answers.

​

What if the forms could not be loaded?
Reload the editor and try opening the picker again. If the problem continues, contact Cademy support with the course name, lesson name, form name and the message shown. A loading error does not by itself establish that the form was deleted.

​

Can learners change a submitted response?
No. The lesson shows their saved response. If an answer needs correcting, they should contact the course provider. Reopening the lesson or changing its progress back to Not Started does not reset the form response.

​

Can I collect file uploads?
Yes, with a General form containing a File Upload question. Feedback forms do not support File Upload. Use Forms: General to set file types, sizes and limits.
